Use one changed procedure and its actual task assignments. Turn the revision difference into an evidence requirement before assessing individual completions.

1. Changed work and chosen assessment
Identify the old and new procedure revisions and the affected step. Describe what a person must now do differently, the learning or assessment required and the impact owner’s rationale. Separate an editorial change from a changed operating task.

2. People, roles and method scope
List each person-method assignment in scope, including temporary staff and recent task assignments. Record exclusions and their reasons. If a person performs two methods, assess each applicable requirement separately.

3. What an assessor must observe
Describe the observable actions, source instruction, exercise conditions and acceptance criteria. For a setup change, include how the person identifies the assigned revision, checks the configuration and responds to a mismatch.

4. Evidence and assessment by assignment
For each person and revision, link the acknowledgement, practice attempt, assessor’s observations and accepted outcome as distinct evidence. Record failed or pending attempts. If earlier evidence is accepted as equivalent, cite the supported equivalence decision.

5. Current readiness and outstanding assignments
Record which person-method assignments satisfy the plan, which evidence or assessment remains outstanding, and who owns the next action. Note changes since the earlier population was reviewed; do not rewrite that historical assessment.
